Thank you for your continued trust in Fountainhead Dentistry. As many of you have experienced, our office has become very busy! In order to better serve all of our patients, we ask that you give notice if you can’t make your appointment. We will implement the following cancellation policy starting on 09/01/2026.

There will be a $50 fee for appointments cancelled with less than 24 hours notice.
This charge is not covered by insurance.

If your appointment is on a Monday, we ask you contact us by 12 PM (noon) on the Friday prior to your appointment.

Please arrive at your scheduled time for your appointment. If you are more than 15 minutes late, you may be asked to reschedule and a missed appointment fee of $50 may be charged.

All of our new patient paperwork is sent digitally prior to your first appointment. If you cannot complete it beforehand, please arrive 30 minutes earlier than your appointment to complete a paper copy.
